We are excited for your child/children to join us at Wasatch Elementary! 

To register, please complete the following documents:

• Student Information Card (Print Version)

• Guardianship Status Form

• Proof of Residency Form (must be verified in the office)

• Media Permission Letter

• McKinney-Vento Homeless Form (if applicable)

• Bring the completed documents to the main office, along with:

  • Original birth certificate of the student

  • Original immunization record

  • Guardian photo I.D.

  • Two proofs of residency
